Nurse Practitioner-In Home Evaluations (Contractor)

Hickory County, NC

The Nurse Practitioner will perform comprehensive assessments, which are consistent with annual wellness visits, in the patient’s home. The Nurse Practitioner works to assess and evaluate the health of our members to ensure they are connected to needed services and support. The Nurse Practitioner follows established clinical and coding guidelines.

Job Description

This is a part-time contract position. Hours are very flexible and can accommodate almost all calendar preferences. This position will require traveling to members’ homes in the Piedmont and Mountain Regions of North Carolina. It is not a remote position.

The Clinician/Provider will have the ability to practice autonomously with expertise in health conditions and documentation. In this position, you will need to complete a detailed evaluation of a member with the assistance of known clinical data. Troy Medicare is focused on improving the quality of life for health plan members through effective care coordination.

In this position, the Clinician will utilize their advanced clinical knowledge and expertise to complete in-home and/or virtual comprehensive health evaluations, capturing an accurate picture of the member’s health including all current, chronic, and historical conditions, identifying gaps in care and uncovering clinical and social determinants that contribute to the management of the member’s health.

The clinician must have advanced medical knowledge, the ability to evaluate the results of questions asked, explain,  perform and interpret various lab and screening tests, analyze potential contraindications regarding medications and interactions with over-the-counter drugs and supplements, ability to determine diagnoses based on supportive evidence and provide additional assessments and determinations of health issues. The clinician will also be evaluating how the assessment findings impact the member’s health, identify risk factors that may exist and how this may impact their current health and need for further evaluation with referral to the health plan’s case management, primary care provider, and/or notifying adult protective services.

Minimum Education/Licensing Requirements: 

  • Graduate of an accredited nurse practitioner program or accredited physician assistant program whose graduates are recognized as candidates for licensure by the regulatory authority issuing the license
  • Clinician with license in good standing in the state where work is being performed
  • Nurse Practitioners and Physician Assistants must have current board certification
  • Must be registered with Medicare

Essential Skills/Experience:

  • Minimum of 2 years of clinical experience working with complex adult populations
  • Minimum 1 year of experience conducting Health Risk Evaluations preferred
  • Experience performing chart audits and educating peers on best practices preferred
  • Ability to document within available EMR and working knowledge of documentation guidelines
  • Adult, Family Medicine or Geriatrics experience preferred
  • Ability to make physical and mental health assessments of the members
  • Ability to conduct and analyze the results of various medical tests
  • Ability to work remotely and autonomously

Compensation

  • $150+ (based on experience) per completed at-home assessment

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. Work involves exerting up to 20 pounds of force occasionally and/or a negligible amount of force constantly to move objects. The use of arm and/or leg controls requires exertion of force greater than that for Sedentary Work and the worker sits most of the time, the job is rated for light work.
